Job Description
The main aim for this job role is to share workload and responsibilitywith General Practitioners, providing a service complementary to GPs andmaking best use of clinical skills within the practice.
To provide high quality, easily accessible service.
Thepost-holder will be expected to work with the Primary Health Care Team toensure a seamless delivery of care within the Practice and to promote apositive image of the Practice to its patients and service users.
Main duties of the job
To provide a holistic, autonomous and clinical nursing service to our patients. Making an assessment of their physical, psychological and social needs, implementing agreed management plan with patients including appropriate prescribing and referral.
Manage patients presenting with a range of acute and chronic medicalconditions, providing subject matter expert advice to patients
