As a Care Coordinator , you'll play a vital role in ensuring the smooth delivery of high-quality, person-centred care. You'll work closely with care staff, the people we support, families and healthcare professionals to ensure services are safe, effective and tailored to individual needs. This is primarily an office-based position with occasional field support when required.
