Compensation
Base Salary: $61,000 – $76,000 (commensurate with experience). Contract position ending July 2027.
Key Areas of Accountability
- Lead a team of Personal Support Workers, Personal Support Coaches, and Care Team Scheduling Coordinators.
- Oversee all performance management activities: performance appraisals, coaching, mentoring, and staff development.
- Implement progressive disciplinary action as required.
- Listen to and address client and family concerns, conveying understanding and exploring solutions.
- Monitor operational performance and key performance indicators to assist operational decision‑making.
- Support QI projects and process improvements.
Required Qualifications
- RN or RPN with CNO registration in good standing.
- Minimum of 3 years experience in community health (preferred).
