Overview

We are seeking a dedicated and creative Diversion Case Manager to lead and expand our Shelter Diversion program. The role focuses on preventing homelessness for young people aged 16‑24 by identifying safe alternatives to shelter stays, coordinating with families, landlords, and community partners, and providing follow‑up case management to ensure housing stability.

Responsibilities

• Engage with young people experiencing or at risk of homelessness to solve housing problems and secure safe alternatives.
• Partner with families, friends and chosen supports to facilitate transitions.
