Job Description
UPMC Community Medicine Incorporated is hiring a Full-Time Certified Registered Nurse Practitioner to help support UPMC Family Hospice.
Hours: Monday-Friday daylight - rotating on call coverage
Location: Lawrenceville and Seven Fields and patient's homes
Position Specifics: Experience in primary care or internal medicine is ideal, care across the life span - experience in Palliative or hospice Is ideal.
Department Details: UPMC Family Hospice is a part of UPMC @ home, where we are committed to providing care through end of life in the home. As an interdisciplinary care team we would collaboratively to support the patient and family unit. The CRNP is a vital part of the care team and can help drive the plan of care, support the team with symptom management visits, help the providers determine eligibility, complete regulatory required virtual/in person F2F visits and provide coverage as the on call provider at the IPU and in the community hospice teams.
