Role Overview
The Dietitian will provide holistic, evidence-based nutrition care to patients and caregivers in the primary care setting.
Services will be delivered face-to-face in clinic settings and/or via approved virtual care platforms with focus on chronic disease management. The role includes supporting and participating in health promotion initiatives alignedwith national strategies such as EatWise and Healthier SG, contributing to continuous improvement in patient outcomes.
Key Responsibilities- Conduct comprehensive nutritional assessmentsand develop individualised, evidence-based nutrition care plans and patient education materials.
- Deliver high-quality clinical nutrition services to support disease management, prevention, and health maintenance in primary care.
