Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.
Serenity Hospice Care is looking for an enthusiastic and organized S ocial Worker for our Monmouth County areas. This position is full time, Monday through Friday from 8:30am to 4:30pm.
Responsibilities:
+ Assist with performing the social services section of the assessment process as well as provide on-going care to patients and their families as needed.
+ Assist with performing bereavement services (calls, mailings, documentation).
+ Provide updates to the bereavement plan of care as necessary.
+ Attend Interdisciplinary Team meetings as needed.
+ Managing and completing all psych-social assessments
+ Educating the patient and family about services
