Looking for an RN Case Manager position in Cambridge, Ontario? We have both contract and permanent openings with options for on-site, hybrid, or remote work.
In this RN Case Manager role, you will be responsible for daily patient monitoring and managing care transitions. You'll work with medical teams and families, ensure accurate record-keeping, and facilitate necessary procedures. Your critical thinking skills will be essential as you determine the best actions based on client policies and needs.
Key Responsibilities:
• Monitor hospitalized and outpatient patients daily
• Liaise with medical professionals for assessments
• Approve defined medical procedures and follow-ups
• Discuss benefit information with patients and families
• Investigate files for proper course of action
Requirements:
• 2 years of RN or Case Manager experience
• Bachelor of Science in Nursing essential
• Certificate of competence as a Registered Nurse
