Overview

Provide complex care and support to new individuals, ensuring all paperwork is in place before initial care delivery, and shadow‑train new support workers. Travel around the North West as required.

Responsibilities

  • Travel around the North West to provide short‑term care and support to new individuals in accordance with their Personalised Plan.
  • Shadow‑train new core care team support workers, ensuring competence for clinical nurse sign‑off.
  • Ensure all necessary paperwork is in situ within the home setting prior to initial care delivery.
  • Assist with initial rostering and coordinating rotas for the core care team.
  • Act as a role model, consistently demonstrating AFG’s values and safe, effective, high‑quality care.
  • Provide health‑related and social support to people with complex care diagnoses (e.g., Acquired Brain Injury, Spinal Injury, Learning Disability, Autism, Mental Health).
