Contribute to newcomer health and settlement as a Case Manager at Sherbourne Health. This full-time role is centered on providing vital support in Southeast Toronto.
As a Case Manager, you will play a key role in ensuring that newcomers can navigate healthcare and settlement paths effectively. This contract role emphasizes building personalized plans, coordinating services, and monitoring clients' success towards achieving their goals in Canada.
Key Responsibilities:
• Assess newcomer client needs and develop plans
• Provide essential referrals to various services
• Collaborate with healthcare teams for comprehensive support
• Advocate for clients in accessing necessary services
• Maintain updated documentation of client data
Requirements:
• Undergraduate diploma or degree in social services
• At least three years of relevant sector experience
• Demonstrated understanding of diverse populations’ barriers
• Excellent writing and presentation skills
