Key Responsibilities
- Providing consultation, health teaching and referral to support improved client outcomes within our programs.
- Providing virtual nursing assessments through the Collaborative Care model; these include nursing recommendations and interventions.
- Assisting with the program development, implementation and delivery of Health Services. This includes assessing and referring, health teaching, policy implementation, recruitment of practitioners, and client follow up.
- Supporting the Nursing team as the program expands and evolves, assisting with the creation of process flows, forms, and service standards.
- Assisting with specialized training for the practitioners providing care within these programs and supporting those clinicians in the expectations surrounding treatment and reporting.
