This is an exciting opportunity to practice full-scope, longitudinal primary care in a supportive clinic environment while making a direct and meaningful impact on access to care in our community.

The Opportunity

We are seeking Family Nurse Practitioners who are interested in building a longitudinal primary care practice in a collaborative, team-based setting.

This opportunity includes:

• Full-scope primary care practice with a focus on longitudinal patient attachment
• Patient attachment supported through the Health Connect Registry and a retiring physician panel
