Extendicare is seeking a dedicated part time Registered Practical Nurse (RPN) to deliver patient-centered care. This role emphasizes effective care plans, communication, and team collaboration.
As an RPN reporting to the Director of Care, you will provide comprehensive, goal-oriented nursing care in line with provincial standards. Your responsibilities include conducting clinical assessments, updating documentation, and directly administering medication treatments. Working collaboratively, you will coordinate care for residents, ensuring their needs are met effectively and promptly.
Key Responsibilities:
• Execute resident's clinical assessments and care plans
• Administer medication/treatments per established standards
• Facilitate communication with residents and families
• Manage care team assignments efficiently
• Respond to changes in residents’ conditions
