Pharmacy Tech – Montpelier Health Centre Job summary  
Montpelier Health Centre is seeking an enthusiastic, well-organised, and patient-focused Pharmacy Technician to join our expanding multidisciplinary team. 
Main duties of the job  
As a key member of the medicines optimisation team, you will: 
Support medicines reconciliation and synchronisation, particularly for patients transferring between care settings 
Liaise with community pharmacies, hospitals, and specialist services to ensure safe and effective continuity of care 
Work in partnership with patients to support appropriate medicine usage and adherence 
Ensure Shared Care Protocols (SCPs) are followed, referring back to specialists where appropriate 
Support the prescription ordering and processing system, ensuring safety and efficiency in handling repeat prescriptions
