Primary Care Physician (MD/DO) – Concierge Outpatient Practice

The Primary Care Physician provides comprehensive, patient-centered health care services within a concierge medicine model. This role focuses on delivering high-quality, relationship-driven care through extended visit times, smaller patient panel sizes, and a strong emphasis on preventive medicine, chronic disease management, and continuity of care.

This opportunity includes inheriting an established patient panel from a retiring physician, offering immediate continuity and a built-in patient base.

**Responsibilities:**

+ Perform routine exams, assessments, and preventive screenings
+ Diagnose and treat acute and chronic conditions
+ Develop and manage personalized care plans
+ Prescribe medications and coordinate treatment plans
+ Educate patients and families on health conditions and preventive care
+ Maintain accurate and timely medical records
+ Collaborate with clinical and administrative staff to ensure optimal patient outcomes
