Job Description

Reporting totheSenior Manager, Recreation and Rehabilitation, theProgram Consultant – Registered Social Workeris responsible forproviding professional leadership and clinicalexpertiseto support evidencebased,residententered psychosocial and rehabilitation services. The incumbentsupportsthe development, implementation, and ongoing evaluation of social work standards, best practices, quality improvement initiatives, policies, programs, and procedures across LongTerm Care Homes and Retirement Communities.

This role is an operational leader who works collaboratively with interprofessional teams and key stakeholders to advance Extendicare’s vision of becoming the best seniors’ care and services organization in Canada. Through mentorship, consultation, andevidence ofinformed practice, the role supports excellence in care, resident wellbeing, safety, and qualityoflife outcomes.
