Job Description

 We are looking for an experienced Relief Support Worker to provide person-centred support to people with a learning disability in Jarrow, South Tyneside. Your responsibilities will include:
 Providing intimate personal care, practical and emotional to people with a learning disability
 Identifying and accessing opportunities with the service users to support them to lead meaningful and fulfilling lifestyles
 Support individuals in the community with various activities. Including- shopping, disco & health appointments
 Collaborating with service users, families, and involved professionals to provide a consistent and coordinated service that meets the needs of service users
 Working as part of a team and alone in service user homes
