The Senior Manager/Manager provides strategic leadership and operational oversight for teams within Integrated Continuing Care Operations (ICCO), driving healthcare initiatives that enhance care coordination between acute and community settings. This role include:
Provide strategic direction and oversight for Integrated Continuing Care Operations (ICCO) teams, including Tiong Bahru Community Health Centre (TBCHC) operations and the EAGLEcare programme, ensuring alignment with organisational objectives and population health goals.
Lead engagement of internal and external stakeholders and oversee team management of projects and resources to achieve desired project outcomes.
Conduct comprehensive assessments of nursing home capabilities and stakeholder readiness to identify critical requirements for successful care integration initiatives, ensuring sustainable and effective implementation of collaborative care models.
