Role & Responsibilities
- Assist the Head and/or Counsellor to attend to referrals for placement, conduct intake and admission interviews
- Use appropriate assessment tools to assess risks, needs, strengths of residents and work collaboratively with the Probation, Child & Adult Protective Services Officers to develop the Individualized Care Plan (ICP) with a view towards reintegration with family and with the larger community
- Conduct in depth biopsychosocial assessment of vulnerable families, design and deliver client centric interventions to vulnerable families
- Ensure that the ICP is carried out with appropriate and sound intervention plans
- Conduct regular meetings with relevant partners and stakeholders to evaluate resident’s progress
