Job Title: Social Worker – Learning Disabilities

Pay Rate: £28.40 per hour
About the Role:

Liquid Personnel is recruiting Adult Social Workers to join its client’s Learning Disabilities Team based in Walsall. In this role, you will support adults (18+) with a diagnosed learning disability to lead safe, independent, and fulfilling lives. This is a person-centred and strengths-based role, requiring collaboration with multiple agencies to meet care and support needs in line with the Care Act 2014.

What will your responsibilities be?

In this role, you will 
Complete Care Act assessments to determine eligible care and support needs for patients ready for discharge.

Develop and implement strengths-based support plans in collaboration with the individual, their family, and professionals.
